Observing a different and increasing need for assistance to the home-bound elderly and their families, the Ageless Foundation supported and assisted in developing a comprehensive home visit program in Manhattan. Specialized geriatric physicians and nurses assist families in numerous interactive difficult-to- access community resources.

The Home Visit Doctor can maximize the amount of home care services available to an individual by coordinating and activating the spectrum of available public, private, and volunteer services. They review insurance and prescription drug plans to assist in the selection of the most affordable medications and treatment programs many seniors are entitled to but may be unaware of, i.e., the best treatment at the lowest price.

This "one-stop-shopping" approach maximizes continuity of care, limits confusion and multiple referrals, minimizes frustrations with "the system," and most importantly, keeps the patient in the home environment, where he or she is most relaxed and comfortable. Frequently, this type of medical-custodial intervention can prevent or postpone moving an elderly person to a nursing home; resulting in significant financial savings and the avoidance of severe emotional trauma.

A frequent phone call program maintains contact with home-bound seniors and can provide advance reminder-notification calls to ensure that home care visits are coordinated. The Home Visit Doctors are on call 24 hours per day and offer liaison medical service at the emergency rooms of Local Hospitals.

Currently, physician participants are availible only in Manhattan. However the Foundation continues to support and persue fundraising to allow continual development of a complete training for dissemination to other boroughs and states.
